Previous GP

We will use this information to contact your old GP practice to obtain your records so that we have the most up to date information about your medical conditions.


Medical Information

Whilst we are waiting for your old medical records to be transferred it is useful to have an idea of any current medical issues and medicines being taken so we can provide you with the best personalised service.








Carers

Knowing this information is useful as people who have carers sometimes need additional support from their GP surgery and we would aim to prioritise you for this.

People who are carers for others are sometimes entitled to additional NHS services which is why this information is also asked.





Cervical screening (smear tests)

People who have a cervix should be offered cervical screening between the ages of 25 and 64. This cervical screening is performed at the GP pratice. Knowing when your last cervical screen was will enable us to offer you your next test at the right time (and arrange for one now if you happen to have missed your last one)

What happens next?

After you press submit this form will be sent to our practice admin team. They will register you with our practice and record the information you have provided in your medical record. Depending on your answers we may then invite you to attend the practice for a new patient check to find out more about your medical conditions, do some routine checks such as taking your blood pressure and maybe some blood tests if needed.

If you are taking regular medication your old GP practice has a duty to provide you with 30 days supply from the date you tell them you are moving practice. We would normally aim to be able to supply you with your regular medications before this point. However if you need a regular medication urgently (within the next 2 weeks) please contact your previous practive regarding this.
